NOTES FROM THE NURSE

All new students must have a current physical dated within one year of the first day of school. Physicals must be provided to the nurse within 14 days of the first day of attendance. Physicals must be on the mandatory NYS school examination form. Many physicians use a template that follows this form which they complete electronically. 

If using your physician’s patient portal, please note that Continuity of Care forms, summary of office visits, and Reports of child in care statements are not physicals, and they will not be accepted.

All new students must also provide their immunization record within 14 days of the first day of attendance.

While these health forms are required within the first 14 days of school, it is highly recommended to turn them in with their registration paperwork, so the nurse may review them and contact you if anything is incorrect or missing.
